ComASUR's Active Commissural Alignment Brings Surgical Principles to TAVR

The ComASUR balloon-expandable delivery system's active commissural alignment capabilities

were highlighted. Unlike many currently available TAVR systems that may rotate unpredictably

while crossing the native valve or during deployment, the DurAVR delivery system allows

operators to actively rotate the valve only after crossing the aortic valve, thereby minimizing

unintended rotational changes that commonly occur while navigating the arch.

Operators explained that alignment begins after crossing into the left ventricle using

fluoroscopic visualization of the radiopaque commissural markers. Small, highly controlled

rotational adjustments are performed until the valve commissures align with the patient's native

anatomy using cusp-overlap imaging.

Importantly, investigators emphasized that once alignment was achieved, the system

maintained this orientation throughout deployment rather than rotating during balloon inflation.

They highlighted this stability as a unique engineering feature that may preserve intended

commissural alignment more consistently than existing balloon-expandable platforms.

This procedural stability was repeatedly described as important for preserving coronary access,

restoring native leaflet orientation, optimizing neo-sinus wash out, and potentially improving

long-term valve function.
