medical trend. UNH's commentary did not indicate a deterioration in performance, as

cost trend was broadly in line with UNH's expectations, and management highlighted the

same areas we've heard from other MCOs (e.g., specialty pharmacy, behavioral health,

and HCBS), with trends somewhat mitigated by measures such as network curation and

payment integrity/fraud, waste, and abuse actions.

• OptumHealth's improvement toward target margins remains a key point in the bull case

for UNH, and 1H performance has been slightly better than management anticipated,

driven by core cost performance (both medical and operational), with the 2H step-down

attributed to normal seasonality as well as the timing of certain restructuring costs.

Management noted OptumHealth entered 2027 benefit planning earlier than in prior

years, with proactive payor collaboration to better align rates and risk. AI efficiencies are

expected to accelerate in 2027/2028. We see these as continued signs of operational

improvement that should support margin recovery for OptumHealth beyond 2026.
