2026 Biopharma Payer Survey: Evolving coverage trends across our universe

dication in rival Camzyos’ label would disadvantage it from eGFR: estimated glomerular filtration rate

achieving preferred status—but some would still need guideline / compendia inclusion. GLP-1: glucagon-like peptide-1

Preference for GLP-1s in new starts in MASH / NASH I&I: immunology & inflammation

About half of our respondents are actively managing the MASH / NASH space, with 84% IgAN: immunoglobulin A nephropathy

preferring patients start on a GLP-1 before Madrigal’s Rezdiffra. Roughly 70% would MASH/NASH: metabolic dysfunction-associated

require >25% rebates to prefer Rezdiffra over a GLP-1, though beyond price, about half steatohepatitis / nonalcoholic steatohepatitis

indicated improved efficacy could be enough to secure preferred access. NCFB: non-cystic fibrosis bronchiectasis

oHCM/ nHCM: obstructive/ non-obstructive

Movement toward broader access of Insmed’s Brinsupri hypertrophic cardiomyopathy

Responses skewed modestly toward less aggressive management of NCFB (60/40), PBMs: pharmacy benefit managers

though 48% lacked sufficient familiarity with Brinsupri to opine on its efficacy. There

SoC: standard of care

also appeared to be willingness to cover Brinsupri in comorbid COPD / asthma patients,

though most would need / prefer label expansion or additional clinical evidence.

Menin management remains relatively open

While 60% had no preference between the menin inhibitors, there was a slight bias

toward Syndax’s Revuforj (24%) over Kura’s Komzifti (16%). Overall, 64% are not

managing the class aggressively—with potential readthroughs to off-label use (the 1L

setting or earlier in the paradigm).
