●40% of Komzifti patients were treated in combination with either
ven + aza (chemotherapy) or gilteritinib (FLT3 inhibitor), which is
consistent with 40% seen with Revorforj. The giltertinib combination
is particularly notable as Revorforj may have a harder time combining
with this drug due to QTc liability.
Road to market leadership and frontline expansion to come
●Kura is planning to build out Komzifti's launch by bringing additional
accounts online (currently at 60). Publication of data (Komzifti + ven
+ aza) and potential inclusion in NCCN guidelines is also expected to
facilitate uptake with some physicians.
●Komzifti's uptake in combination with gilteritinib is viewed as leading
indicator for commercial success:
○1. Kura has not shared any data of Komzifti in combination with
gilteritinib yet so physicians must feel comfortable enough to
combine it.
○2. Revuforj and has a black box warning for QTc prolongation
which possibly prevents it from being combined with
gilteritinib.
●The Ph3 KOMET-017 trial combines two frontline trials in one which
enables every opened site to enroll patients into IC and non-IC trials.
Especially the non-IC trial has been enrolling faster than anticipated
though Kura remains in its initial 2028 top-line data guidance.
Data updates at EHA and ASCO should continue to de-risk Komzifti in
frontline AML and darlifarnib, respectively
●Komzifti's efficacy data in combination with IC improved on CRc in
NPM1m (96% vs. 93%, N=49 vs. N=44) and KMT2Ar (90% vs. 89%, N=50
vs. N=27). The median duration of CRc was NR in NPM1m and 11.2
months in KMT2Ar. Kura is guiding to extended follow-up with ~16
months (likely in NPM1m setting).
