Moderate TAVR Enthusiasm?
The Wolfe Byte
15 pages enclosed previewing PROGRESS. It’s definitely a ‘potential big TAM unlock’ kind of catalyst. Is the trial
a layup? No. But is there a path for this study to hit and the moderate opportunity to develop generally? Think so,
yes. We conclude glass half full.
PROGRESS trial to read out this fall at TCT which is scheduled for October 31-November 3. With juicy med tech
catalysts scarce, and this one on its surface potentially big, we dig in on the early side. The study is 750 patients with
moderate aortic stenosis and at least one high risk feature randomized to TAVR or surveillance (i.e., no intervention) with
primary efficacy endpoint a composite of mortality or heart failure ‘event’ with or without hospitalization at two years.
Why is EW running this trial? To this point patients with moderate disease have generally been ‘surveilled’ – i.e.,
watched, not intervened upon. PROGRESS exploring whether for a sub-set of moderate disease it makes sense to
intervene.
So…why care? It’s a ‘potential unlock of a big TAM’ kind of catalyst. The literature is clear – the moderate aortic
stenosis prevalence pool is approximately 2x the size of the severe disease population. What’s also clear? Our Structural
Heart trial benchmarking work indicates PROGRESS enrolled quickly and efficiently – in the zone of the original TAVR
trials – validates it’s a big pond with lots of fish that has never had a boat on the water. Med tech 101? Like big TAMs,
cannot lie.
