Mgmt was emphatic that prescribers do not compare PANSS effect sizes. Of approximately 50,000

US antipsychotic prescribers, the majority are in primary care or pediatrics, and fewer than 100 KOLs

can cite specific effect sizes from clinical trials. Caplyta (d=0.3) represents the lowest bar for an

approved antipsychotic, and the atypical class as a whole averages approximately 0.3 effect size.

Cobenfy's 0.6 to 0.7 is at the high end (note 0.7 is from Phase II, not Phase III), but real-world

perception of its efficacy is shaped more by tolerability-driven adherence than by the clinical trial delta.

If MPLT demonstrates a slightly smaller effect size than Cobenfy (e.g., 0.5) but with meaningfully better

tolerability, mgmt believes this will not matter commercially because prescribers will judge based on

their own anecdotal experience with patients, not side-by-side PANSS tables.

Expectation Bias and Functional Unblinding in Schizophrenia

MPLT acknowledged that expectation bias is a real consideration for muscarinics in schizophrenia,

but does not consider their fast trial enrollment to be a result of this phenomenon. Rather, MPLT

attributed fast enrollment primarily to (1) entering the clinic during a quiet competitive period with few

competing inpatient schizophrenia trials, and (2) the strong site relationships built by MPLT's in-house

monitors. MPLT's in-house monitoring staff are initially and continuously trained extensively against

expectation bias, including nocebo/placebo response training.
