expand TNS market; sees room for multiple vendors; 4) in stones, thulium lasers
and flexible ureteroscopes are key innovations; ESWL vols low, but durable.
In BPH, Dr. Weiner walks pts through their options a full decision tree depending on prostate size,
comorbidities and goals of care. Ultimately, he views TURP-like resective procedures as the gold
standard and noted Aquablation and HoLEP vols are growing at Columbia. In his practice, he
performs bipolar electrovaporization and has historically performed BSX Rezum and UroLift (UL),
which he views as appropriate for prostate size range 40-80 grams. He noted Columbia's UL vols are
declining due to a growing number of competing interventions. He categorizes UL/Rezum together
in the MIST category and while he questions their LT efficacy, he sees continued broad appeal
of the MIST category (prostate size sweet spot 40-80 grams) due to marketing and faster pt
recovery. The doc noted Optilume (Laborie Medical Technologies, private) balloon impressed him,
esp for strictures; iTind and Urocross Expander (Prodeon Medical, private) implants have a place
for higher-risk/anticoagulated pts.
Dr. Weiner's commentary was mixed for Hold-rated PRCT (Mike Sarcone), which he noted works
