DTC GLP-1 Channels Could Pressure RADL; Lilly Tactical Replication Is the Key
Risk. Novo Nordisk’s launch of NovoCare Farmácia in Brazil introduces a potential
alternative channel for GLP-1 distribution, but the near-term risk to RADL appears
manageable unless Lilly adopts a similar model. According to Panorama
Farmacêutico, NovoCare Farmácia allows patients to buy Ozempic, Wegovy, and
Rybelsus online with digital prescription validation. Novo is positioning the initiative
as a pilot and complementary channel, rather than a replacement for pharmacies,
and is using a licensed pharmacy partner to comply with Brazilian regulation.
• The model is not easy for pharmaceutical companies to scale broadly.
Brazilian regulation still requires a licensed pharmacy partner, limiting true
direct sales through open marketplaces such as Amazon or Mercado Livre. In
addition, pharmacy traffic is supported by basket economics: consumers
often purchase multiple products per visit, which reduces the attractiveness
of bypassing drugstores for many drug therapeutical areas.
• GLP-1s are a more credible exception. These drugs are high-ticket,
prescription-driven, and often purchased as standalone items, making direct-
to-consumer economics more compelling than in lower-value or basket-
dependent therapeutical areas. If Novo’s alternative channel offers better
pricing, convenience, or adherence support, it could divert some GLP-1
volumes away from traditional drugstores, creating an incremental headwind
for RADL.
• The more material risk would be replication by Lilly. Novo’s GLP-1
portfolio is likely to become less differentiated as Lilly gains share and
