always been intentional. The problem was throughput and

month lag before touching a practice, stacked up technology and
analytics issues faster than they could be absorbed. The model is not
broken and can be fixed with operational discipline: slow growth,
integrate, and let the proven playbook work.

typically take ~two years due to risk-adjustment and claims lags.
