(617) 345-4042 akassin@btig.com WHAT YOU SHOULD KNOW: Eosinophilic esophagitis (EoE) is a chronic, allergicBIOTECHNOLOGY condition driven by a Th2-dominant immune response. Considering Dupixent is the
only approved targeted therapy for EoE and likely generates over $2.5B/yr in EoE alone
(2x the annualized price vs. atopic dermatitis due to more frequent dosing), we thought
a deep dive on the indication made sense. Our KOL checks support high receptivity
for alternatives to Dupixent (dosed QW in EoE) with lower injection burden at parity
or better efficacy; based on a review of the treatment landscape, zumilokibart (fkaINDUSTRY APG777) looks well positioned to command significant market share at Q3M or better
dosing intervals in EoE. Tezspire could be appealing vs. Dupixent although we wonder
how competitive QM would be vs. a Q3M anti-IL-13 Tx option, and the effect size vs.
Dupixent seems reasonably uncertain ahead of Phase 3 data in 2H26 (interestingly not
preceded by any public clinical data, as far as we can tell). Current frontline treatmentREPORT options (e.g., proton pump inhibitors [PPI], steroids, food elimination diets) leave
meaningful unmet need, and EoE's high comorbidity rates with asthma and atopic
dermatitis favor broadly labeled therapies - considering how strong Dupixent's efficacy
is, the goal for most prescribers is to get patients on Th2 inhibitors as fast as possible.
In this report, we discuss EoE pathophysiology, diagnosis, current treatment patterns
