Improved Neosinus Washout Addresses Emerging Durability Considerations
A recurring theme throughout the presentation was the increasing recognition that valve
performance extends beyond simple transvalvular gradients. Investigators emphasized that
neosinus washout has emerged as an important design consideration because stagnant flow
within the neosinus may contribute to leaflet thrombosis and long-term valve degeneration.
According to investigators, the X4S incorporates additional design modifications specifically
intended to optimize neosinus flow dynamics while maintaining consistent leaflet stress across
all deployment diameters.
A New Delivery System Designed Around Procedural Precision
Sapien X4S is delivered through the newly developed X4S delivery system, which retains much
of the familiar ergonomics of the Commander platform while incorporating several workflow
improvements.
The valve now arrives pre-mounted directly on the deployment balloon, eliminating the need
for in-procedure valve loading and simplifying procedural preparation. The delivery catheter
incorporates a flex wheel that facilitates navigation through the aortic arch, while retaining the
familiar fine positioning control used during deployment.
Perhaps the most notable addition is a dedicated valve rotation control, allowing near one-to-
one rotational adjustment during implantation to intentionally achieve commissural alignment.
Operators described the system as intuitive and straightforward, with simple rotational
adjustments performed prior to valve deployment using fluoroscopic commissural markers.
