Exploratory PROs were also supportive and fit the company’s broader message that fatigue and

cognition matter commercially in central disorders of hypersomnolence. PROMIS-Fatigue was

nominally statistically significant vs. placebo at week eight at the 14 mg and 18 mg doses, with

improvements seen as early as week two and mean values essentially within the normal range

by week 8, with continued benefit in the OLE. BC-CCI scores measuring cognition also improved

as early as week two and continued through the OLE, with nominal statistical significance

at week eight for the 18 mg dose. These endpoints remain exploratory; however, they are

directionally important given management’s repeated emphasis that alixorexton may have

cognitive effects even in patients with intact orexin signaling, and support the overall profile of

alixorexton.

Dosing Flexibility Remains Central To The Competitive Story

Dinner discussion reinforced that dosing flexibility is likely to be one of the most important

differentiators for alixorexton. Management characterized the Ph. III program as designed

around once-daily anchor doses with the option to evaluate split dosing in selected patients,

reflecting the heterogeneity seen across NT1 and NT2. The company believes it has a framework

for dosing across variable patient biology and exposure, and noted that it has room in the

dosing range, having gone higher in healthy volunteers. Management also emphasized that

MWT optimization is only one part of the competitive story, with ESS normalization, full-day

coverage, and patient-relevant outcomes likely to matter more in real-world use.
