ESTA Has Developed A Disruptive Breast Implant Technology Platform, Not Just “Another Gel”

The surgeons repeatedly emphasized that Motiva is meaningfully different from legacy

implants, not merely a marginal iteration of existing silicone devices. The discussion focused

on the implant’s softness, adaptability, rheology, gel behavior, and surface technology. One

surgeon with an engineering background described Motiva as a “complete departure” from

existing implants, noting that its material properties and feel were fundamentally different from

competitive devices. Another highlighted that the implant’s safety profile and natural aesthetic

enabled surgical technique changes that were not feasible with prior implant options.

A particularly important clinical and commercial implication is the shift from submuscular or

dual-plane placement toward prepectoral or over-the-muscle placement. Historically, several

surgeons preferred submuscular placement because they were concerned that legacy implants

are associated with higher rates of capsular contracture and looked or felt less natural in the

prepectoral position, particularly in lean patients. Motiva has significantly reduced the capsular

contracture concerns due to the record low 0.5% rate demonstrated in the PMA study and has

been deemed revolutionary. A significant number of patients in the PMA trial had prepectoral

Motiva implants and the 0.5% capsular contracture rate was experienced by this subgroup as

well. One New York surgeon with a lean patient population said Motiva allowed the practice to

move almost exclusively over the muscle, reducing downtime and making breast augmentation
