We hosted Dr. Ana Franceschi, the Director of Molecular Neuroimaging at
Northwell. Key takes: AD care is undergoing rapid transformation, with disease-
modifying therapies driving step-function growth in diagnosis and imaging,
while neuro PET has risen to >2k scans in ’25 vs ~200 in ’18/19 and sustaining
over next 5yrs. Therapy adoption remains modest (~40 pts) amid ARIA
concerns but improving. BBM expands funnel, PET remains required, with
upside from tau.
Dr. Franceschi highlighted the rapid evolution in AD and dementia care following the introduction
of disease-modifying therapies, with Leqembi (Jan ’23) and Kisunla (July ’24) representing the first
meaningful advances in decades.
These anti-beta amyloid therapies are driving a sharp increase in neuro PET scans, rising from
~200 annually in 2018/19 to >2k in 2025. She expects this growth to continue at a similar
pace over the next 5yrs as earlier diagnosis and treatment pathways become more embedded
in clinical practice. Tracer selection today is influenced by reimbursement and site of care,
with Amyvid (LLY) more commonly used in hospital settings due to higher reimbursement, while
Neuraceq (LNTH) is more prevalent in outpatient settings.
PET scans have largely displaced lumbar puncture in clinical practice, and the emergence of blood-
based biomarker (BBM) testing (May ’25) is further reducing diagnostic friction by providing a low-
cost, scalable entry point that brings more patients into the funnel.
However, Dr. Franceschi does not expect BBM to displace PET in the near term, as current
