Orforglipron was well-received by prescribers in the ACHIEVE symposium, encouraging our already optimistic view.
The discussant drove home its better glycemic effect and weight loss, its similar or better CV risk factor effect and
ease of use versus approved agents.
Setting the stage for orforglipron in Type 2 diabetes. Orforglipron ("orfo," branded Foundayo) was recently approved
by the FDA as the first small-molecule GLP-1 inhibitor for obesity (see our thoughts here). Earlier today, we received
our first comprehensive look at orfo in Type 2 diabetes (T2D), including a head-to-head against Novo's (NOVOB) oral
semaglutide. Looking forward and following orfo's solid results from ACHIEVE-4, which reaffirmed its cardiovascular
and overall safety profile (our thoughts here), LLY plans to submit Foundayo to the FDA under the Commissioner's
National Priority Review Voucher (CNPV), the same expedited approval process where it garnered its first approval (see
our approval note here), by the end of 2Q26.
ACHIEVE-2 results came in as expected. ACHIEVE-2 is a 40-week, open-label study investigating orfo (2.5, 9, and
17.2 mg) compared to AstraZeneca's (AZN, not covered) dapagliflozin 10 mg in participants with T2D and inadequate
glycemic control on metformin. To provide some context, patients had an A1C between ≥7.0% and ≤10.5% (baseline
level was 8.1% at screening) and a BMI of ≥23 kg/m2 (baseline was 32.6 at screening).
●Efficacy: Using the treatment-regimen estimand, at week 40, patients on orfo showed HbA1c reductions of -1.2%,
