Metastatic Castration-Resistant Prostate Cancer (mCRPC) Overview
Metastatic castration-resistant prostate cancer (mCRPC) represents an advanced stage of prostate
cancer defined by disease progression despite sustained androgen suppression. Since prostate cancer
growth is largely driven by androgen receptor (AR) signaling, androgen deprivation therapy (ADT)—
via medical or surgical castration—is a foundational treatment and can control disease for prolonged
periods. However, resistance inevitably develops, leading to castration-resistant prostate cancer
(CRPC), characterized by tumor progression despite castrate testosterone levels (<50 ng/dL).
CRPC can present as either non-metastatic (nmCRPC) or metastatic (mCRPC), with the latter defined
by radiographic evidence of distant disease. mCRPC is associated with poor prognosis, limited duration
of response to available therapies, and accounts for the majority of prostate cancer-related mortality.
